STATEMENT – consent to the processing of personal data

By signing the statement, the client grants consent or express consent to the Pension and Disability Insurance Institute of the Republic of Slovenia (hereinafter: Institute) for the processing of the submitted personal mobile number and personal e-mail address for the purpose of the safe receipt of the Institute’s documents – the implementation of the SMS service, and agrees:

• to have the Institute submit the notification of the safe receipt of the client’s documents on the web to the submitted e-mail address
• to have the Institute submit a code for the safe receipt of the client’s documents on the web to the submitted mobile phone number via the SMS notification service

and confirms that they are aware that:

they may, at any time, withdraw the consent to the processing of the submitted personal data – mobile phone number and e-mail address (https://www.zpiz.si/wps/portal/HP/oddajaevlogjavno?sfVloga=020-28-1-J&sfJezik=EN);

• the Institute shall process the submitted personal data exclusively for the purpose for which they were submitted and until the withdrawal of the consent for the purpose of processing;
• the Institute shall protect the submitted personal data and ensure and perform the necessary safety procedures and measures in line with the applicable regulations on the protection of personal data, namely the General Data Protection Regulation (GDPR), the Personal Data Protection Act (ZVOP-1), and the Institute’s internal Policy on the protection of personal and other protected data, which is accessible on the Institute’s website;
• concerning their personal data, they have the right to be informed, the right to amend, correct, restrict the processing of, the right to delete, the right to object to and the right to transfer such personal data, including the right to file a complaint to the supervisory authority and the right to judicial protection of individual rights.
